"Ground"

I get that ground shadows and reflections are cosmetic for viewing -- it seems that when I toggle it on/off, the ground seems to switch depending on the current orientation of the viewing window.
I can sort of figure out how to get the reflection/shadow where I want, but is there a formal definition of how the "ground" is determined when you toggle the shadow/reflection, so that I don't have to hunt around?

Sorry I can't help you out. I have those options turned off and have Ambient Occlusion disabled in 3D in my System Options. I find them to be annoying and not very helpful during the design process. If I want reflections, ground shadows and AO, I have KeyShot to render any images with them.
