I uploaded VS2008 project which created ExtThrd.exe.
For those who wants to have API skelton project for MFC based application, this could be of some help.

BTW,VS2008 standard costs you some $220 to $230 only. And it comes all things you need.
I don't think it is too costly, considering of the capability of creating so many type of applications. I bought this with the firms budget but I might as well buy when next version comes.
